Please bring in or email the completed and signed Patient Information and Consent forms to/before your first meeting. To print the forms, please left-click to enlarge, then right-click to save to your computer. Then open the saved files from your hard drive, complete and print. Make sure you scan the QR code for the HIPAA compliant APP - this is the technology that always give you high quality care and access to your pediatrician. Whether your little one is too sick to make it to the office, you are on vacation, your child is at college or it is the middle of the night, we will be there for you through our state-of-the art telehealth system.
